Please fill out this form as completely and accurately as possible so we can get to know you
and your pets before your visit.
A non-refundable deposit is required for all appointments and surgeries. Appointment cancellations require a 24 hour notice to retain the deposit as a credit on your account. Surgery appointments cancellations require a 48 hour notice to retain the deposit as a credit on your account. Should cancellation occur outside these requirements, the deposit will be forfeited.
Appointments are preferred and the best way to reduce wait times but walk-ins are welcome with up to a (2) two hour wait. A $15 walk-in fee is required for each pet and no more than two (2) pets at a time can be brought in for a walk-in. We may pause walk-ins if wait times are too long.
